Description: The Targeted Case Manager coordinates the services needed for the child, family, foster parents and other caregivers; working towards the goal of maintaining and strengthening families. A Targeted Case Manager possesses the ability to interact professionally with other agency personnel, community providers, GALS, attorneys, judges, school personnel, etc. Targeted Case Manager advocates for clients at court hearings, case reviews, staffing, IEP meetings and other interactions as required. A Targeted Case Manager prepares service plans, individual assessments, daily progress notes, social service applications and other required communications
Job Requirements:
Minimum Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, Criminal Justice or related field and one year experience working with children with serious emotional disturbances or bachelor’s degree and three years full time or equivalent experience working with children with serious emotional disturbances

Targeted Case Manager Responsibilities include:
Coordinate the services needed for the child, family, foster parents and other caregivers; working towards the goal of permanency.
Communicate with DCF, family, etc. regarding progress in care and facilitates a team approach in the development of individual service plans
Counsel client according to service plan goals and objectives. Link and assist client with community resources related to service plan goals
Conduct home visits on individual child and family's weekly
Attend bi-monthly supervision meetings on all clients in order to review client’s service plan progress
Attend and participates in scheduled trainings
Attend meetings as requested with a multidisciplinary team, education officials, mental health providers, referral source and connected social service agencies within the community
Title:TCM Supervisor
Description:
This is a full-time position that is responsible for the provision and coordination of Targeted Case Management services to clients. Supervises a competent unit of Targeted Case Managers.
Primary work hours are Monday through Friday from 9:00 am-5:00 pm with an hour of lunch. The hours are flexible and depend of the need of the business. Therefore, the supervisor may be contacted prior to 9:00 am and after 5:00 pm.
The TCM supervisor is responsible for training and supervising TCM staff and for consumer and systems advocacy.
This position is also subject to criminal and abuse background checks.
Salary:Commensurate with experience
TCM Supervisor Duties and Responsibilities include:
Program Supervision and Coordination: Responsible for the day-today supervision, coordination and implementation of program activities for the purpose of efficient and effective service delivery. Monitors the compiling, maintenance and review of necessary client records to assure compliance with county and state regulations. Ensures the preparation and maintenance of statistics and reporting.
Staff Supervision and Evaluation: Provides ongoing supervision to program staff. Completes staff performance evaluations and implement agency personnel policies and procedures as necessary. Assures the timely and accurate staff submission of assessment, service plans, progress notes/documentation and billing reports as required. Assists in interviewing and hiring of TCM staff in conjunction with the Chief Executive Officer.
Staff Development and Training: Develop monthly and bi-weekly staff development/training plans for TCM unit based upon the individual strengths/areas of weakness of each case manager.
Program Development and Planning: In conjunction with the CEO, provides direction, leadership and training to staff in reference to program planning and development in order to most effectively meet client and overall agency needs.
Budget and Fiscal Accountability: Remain familiar and up to date with program budget, helps to maintain cost effective program operation.
Community Education and Public Outreach: Participation in public outreach and community education activities as required. Performs public education activities within the community to gain acceptance and understanding of TCM services and other agency programs.
Facilities Management: Maintains program facilities in accordance with federal, state, county and local regulations.
Qualifications:
A bachelor’s degree from an accredited university or college and five years of full time or equivalent case management experience serving the target population and; Has completed or agreed to complete AHCA –approved mental health targeted case management training within three months of initially supervising Medicaid services.
A master’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education , or in a related human services field and three years of full time or equivalent professional experience serving the target population.
Title:Billing Specialist
This is a full-time position that is responsible for performing billing functions relevant to Medicaid and other medical providers in a manner that meets or exceeds key performance criteria.
Primary work hours are Monday through Friday from 9:00 am-5:00 pm with an hour of lunch.
This position is also subject to criminal and abuse background checks.
Salary: Commensurate with experience. Billing Specialist Duties and Responsibilities include:
Processes billing transactions in a timely and productive manner and meets production goals.
May make calls to provider agencies to follow-up to ensure payment of claims.
Adheres to policies and procedures as detailed in the company’s employee handbook.
Follows all departmental policies and procedures.
Keep supervisor informed of any work problems and issues.
Setup and help maintain personnel files.
Maintains professional and technical knowledge by attending educational workshops, training, etc.
Audit TCM files ongoing and before discharge
Access the Medicaid web portal and secure username and password.
Performs other duties as assigned
Qualifications:
A high school diploma with one year experience working in high volume billing, preferably with Medicaid.

Description:
This is a full-time position that is responsible for the day to day coordination and provision of the targeted case management staff and clients.
Primary work hours are Monday through Friday from 8:00 am-5:00 pm with an hour of lunch.

Office Administrator Duties and Responsibilities include:
Answers the telephone and provides information by answering questions and requests.
Set up client files for target case managers and ensure compliance with Medicaid.
Verify the client’s Medicaid eligibility.
Access and maintain the Medicaid web portal.
Setup and help maintain personnel files.
Maintains professional and technical knowledge by attending educational workshops, training, etc.
Schedule meetings and staffing for TCM’s and administrative staff.
Coordinate services between TCM and other community providers
